The history of racism traces its roots back to the times when skin color really mattered. A few centuries ago it was the main criteria according to which people were forced to judge each other. The skin color was determining their social status, lifestyle and the attitude of the surrounding society. Also, it used to be an unwritten law during elections, job interviews, etc., to discriminate the blacks, which caused a full-scale limitation of their rights. In fact, in modern society nothing has dramatically changed in the perception of the racial diversity. The only change that has come into being is the difference between light-skinned and dark-skinned black people. It has been noticed that people with light hue of skin have more privileges than those who can be described as the African Americans or the Blacks. This phenomenon is called colorism (however, for some reason Webster’s Dictionary doesn’t indicate this term) and denotes a type of racial discrimination towards the dark-skinned while their close counterparts — the light-skinned people are treated more respectably at all levels of social system in the USA and other countries. It can be explained by the fact that although there exists a great variety of anti-racist movements, people unconsciously refer to the old stereotype that white skin is an ideal color for a human creature (together with blue eyes, blond hair and thinness) and is taken as success. Consequently, the rest has to either change their color as Michael Jackson did or reconcile themselves to the circumstances of reality. Note, that colorism is not only associated with the blacks. Unfortunately, this notion as well concerns communities that differ from the white race. Therefore, the Japanese, the Chinese, the Arab, the Native American people are also to be â€Å"blamed†. Desire to match the â€Å"ideal† pushes them to absurd: Arabs resort to the help of the surgeon who will refine their nose shape, Japanese do the same in order to change the color and shape of their eyes, etc. People from all over the world have established all kinds of organizations, clubs, and associations, where they assemble to discuss urgent problems of the blacks to protect their rights and work towards the improvement of the their life standards. Among the indicated above groups it’s possible to distinguish some Black organizations such as â€Å"The Blue Vein Society† where the black person should have his blue veins clearly seen from under the skin in order to get a membership. Another association would not allow the black participate in its activity unless his skin hue is no darker than a brown paper bag.   Therefore, individuals with lighter tone of the skin are considered to be closer to the white race and are treated accordingly. The issue of the skin tone has been vividly discussed in mass media but unsurprisingly enough has never become a subject of hot debate among the authorities. In 1920s a well-known American black female author Nella Larson decided to write a book, which would kodak the influence of the color tone of the blacks on their relationships among each other as well as social attitude and life opportunities in general. The book â€Å"Passing† was published in 1929 and received favorable reviews from the majority of readers. During the Harlem Renaissance era staring at the end of WWI and lasting till 1930s, those having the light hue of the skin have considered themselves almost white easily getting their privileges. As for the blacks, they have come to the point where the society forced them to hate themselves for their skin color. Thus, the main problem of the book seen from various angles is colorism. It would be wrong to consider colorism less essential than racism as, in fact, it is even more dangerous as it provokes tension inside the black communities who ought to preserve their identity forcing people with darker skin to feel as if they were less valuable, unimportant human beings having minimum rights and benefits. The recognition of the notion of colorism assists us in dividing the negative phenomenon of racism into several separate problems that therefore can be much easier to solve. The book â€Å"Passing† depicts the story of two women Irene Redfield and Clare Kendry who have not been in touch for a long time, but reunited already having husbands, children and their own lifestyles. Both of them are light-skinned black females, but Clare was the one who has been passing as a white woman during all of her life in spite of the fact that her husband was a violent opponent of the blacks. Through the whole book one can observe a debatable notion whether it is worthwhile having a light tone of a skin to identify oneself as the black and to refuse from the advantage an American white citizen receives on regular basis or to perceive oneself as a white person and to try to penetrate into the white society with all its opportunities and â€Å"equal† rights leaving behind black culture and identity. Irene opposes the idea of abandoning her black roots and subconsciously takes pride in her origin. Being strongly attracted to Clare, she, however, disapproves her behavior in terms of endeavoring to reach the white upper crust. Interestingly enough, Irene’s character doesn’t have a strong incentive in life. Her existence amounts to nothing more than her family and setting up charity activities for the local cream of the crop. She assumes that her light skin allows her to look down on her dark-skinned companions. This can be clearly observed in the dialog between her and Clare after the latter has spent some time chattering with two Irene’s black servants Zulena and Sadie leaving behind general opinion. (Larsen 79). With an ulterior motive, Larsen chooses to place two black women in the role of servants in particular. It is difficult for Irene to overcome the stereotype of the racial diversity and to lower herself to the level of black low-income servants who nevertheless along with their mistress belong to the same group of the blacks. Nella Larsen in her book â€Å"Passing† brings up the gender issue among those willing to escape from their black roots and become one of the whites. This is vividly described at the beginning of the book while women are having a talk about Clare’s friend Claude Jones who used to be the black but has passed and turned into a Jew. The idea of his break-in doesn’t arouse a protest or indignation among the young ladies; moreover, they consider Claude’s abandonment of his origin a weakness that can easily be forgiven and forgotten: â€Å"Oh, he’s a scream all right†¦ Still, it’s his own business† (Larsen 37). At the same time, women are looked at as traitors trying to cross cultural and skin color lines. Applying theory in practice, image makers unnoticeably turn the audience away from the dark-skinned by putting an image of a black male, not on any account a black woman, into the movie and making him fall in love with a white girl or in very rare instances a mulatto girl who in future will give a birth to less darker generation. Larsen also emphasize the role of man by describing Clare’s husband John Bellew who was an ardent racist and an enemy of the blacks. John Bellew becomes a personification of racism, not mentioning colorism, and places his convictions higher than his love for Clare. His belief in Clare’s real origin start growing very fast revealing the truth of the racial issue of his wife as the book goes on. Irene finds herself under the tension of both Clare and John who keep constraining her to pass. Who knows what would happen if John had time to talk to Clare before her death? Divorce? Soul-searching? In any case, nothing has higher price than life, particularly the hue of the skin. But Clare suspects that there is only the slight chance that her husband would change his attitude towards the blacks. Throughout the book Clare is a devoted follower of the idea of passing as she oftentimes describes the positive sides of the world she is living in to Irene. However, later on she starts complaining about the awfulness of her state and tells her friend that ultimately she would be happier and safer than herself. Unfortunately, the problem of skin tone differentiation remains of current importance even nowadays, in spite of all the technological advances and scientific progress, which seemingly have to contribute to the development of moral values. Basically, the reason lies in the human nature, which cannot be in any way modified or suppressed for a long time. According the study of the PhD student Mr. Michael Harrison, the issue of colorism has a great impact on a business sphere. He claims, for instance, that when two black people with equal experience, talents and personal characteristics are applying for a job, the one who is the light-skinned black individual will most likely occupy the desired position, no matter how sad it is. Plenty of people are making a statement regarding being not implicated in racism, but whenever a conversation deals with colorism and the topic of light-skinned and dark-skinned black individuals reaches the boiling point they shade. Together with mass media representatives they are playing a strategic game where they refer to such celebrities like Mariah Carey, Van Diesel, and Halle Berry as the black, purposefully leaving the fact that these famous figures are light-skinned black people without consideration. This policy shapes the worldview of how the black person must look like and creates an image of exotic appearance of the light-skinned individuals. This results in a new different approach toward those people whose skin is of ebony color. Obviously enough, they are most likely not to get a proper education, descent job and all those regular amenities of life taken for granted by white citizens. Experts say, that in average 70% of dark-skinned black African Americans tend to have menial jobs more often than their light-skinned partners as the last ones make 30% of lower-paid salaried workers. Overall, social opinion served through media has become so powerful and influential lately, that it’s hard to think about the future. Try, for example, to imagine a light or a dark-skinned angel from the top to the bottom, his face expression, hairstyle, clothing, etc. At best, you will get an image of the Angel of Death. Or an Assistant Professor of Sociology at Morehouse College Larry D. Crawford in his study (1997) asks the audience whether they have ever thought why â€Å"does the general complexion become darker and darker when you travel from upper to middle to lower class African communities? Why is it that most homeless men are dark complexioned? Why do we become affectively shocked or disturbed at the sight of a light complexioned homeless man? Why does the former seem more natural and the latter utterly out of place? Why is it that most homeless women are also dark complexioned?†Ã‚   He also refers to the fact that the majority of slaves were dark-skinned black people as the argument of a great difference between the latter and those who had light tone of skin. Is it possible to fully realize an enormous size of the problem of skin color? Would it be better if our mankind was created color-blind? Is there any excuse why individuals with white skin color making 10% of world’s population rule the world and establish priorities for others? Is it possible to explain this phenomenon to our grandchildren? Who is the one to stop the on-going mutual misunderstanding reflected in so many generations of the whites and the blacks, both with light and dark hue of skin? It goes without saying that during a course of the years the issue of racism and colorism has been relevant globally and constantly. History should have taught us a good lesson about the consequences of human distorted apprehension of skin hues through worldwide slavery, Civil War and numerous unknown historical events. Opposites and Paradoxes in King Lear Essay

The sequence of oppositional characters and motifs in the play bring about in the audience a sense of the corruption of principles that beset the protagonists of the play. With this sense of opposition comes a strong sense of the duality within the play seemingly centred on the Epodoclean theory of a â€Å"world governed by the contrary forces of love and hate. Though this is not unusual for a stage production, McAlindon believes that when the bond of opposites that constitutes the natural order of â€Å"revolt against limit and fly to extremes. † This can be seen in the characters foremost as the sons of Gloucester as well as the daughters of Lear are directly opposed to each other. Indeed it is in the internal nature of Lear that this is focused most powerfully as his beliefs in love and kindnesses are offset by the egocentric and chloric feelings that dwell within his heart. It is mainly from the character and fate of Lear that the true extent of the breakdown of nature can be seen as within the space of two weeks he has sunk from kingship to a world of destitution and poverty as he suffers at â€Å"th’ extreme verge† in his relationship with his family. What is most tragic in relation to Lear though is his rediscovery of Cordelia before the heart wrenching death she endures as he is thrown from the heights of grief before his heart gives way under the strain of ecstatic joy. But while the emotional converses that Lear endures are tremendously powerful they are not the only matters in opposition throughout the play. There can be seen in the various settings of the play a number of thematic oppositions, with the most apparent being the contrast between the nocturnal and gloomy castle of Gloucester as opposed to the serene Dover fields where Father and daughter are reunited, where love opposes strife. Indeed there are a great number of inversions that apply a new number of possible thoughts to the understanding of the play. Lear’s sufferings are completely opposed to the more typical tragedies of the Shakespearean era where there was a distinct separation between the suffering of the social elite and â€Å"the low and the ludicrous† in the principal of the Senecan school of thought. In King Lear though it would appear to be the Saturnalism theories that prevail as the positions of the lowest are inverted with those of the highest, as Lear takes the place of his fool in declaring the unpalatable truths of the world in his madness, adopting a sense of tragedy in the manner in which this is done. There are none more demonstrative of inversions than the antonymic nominalism that occupy the play with the most pathetic being Gloucester’s praise of Goneril and Cornwall, whereby his loyalty to the king becomes â€Å"treason† whereas Edmund’s betrayal is described as a show of â€Å"loyalty. † But more than this it is a key illustration of the wickedness of protagonist such as Goneril who condemn â€Å"harmful mildness. † This sense of paradox is prevalent mostly in the evil party where it comes to signify a moral and social inversion of a rational order of things. In contrast to this a positive paradox comes to represent a renewal through destruction and a discovery though loss, most notably seen in the increase of France’s attraction for Cordelia following her rejection by Lear as she becomes an â€Å"unprized precious maid,† becoming â€Å"most choice, forsaken† as the isolation of â€Å"forsaken† seemingly highlighting the paradox. What is more is that a sense of pathos is granted through this as Lear’s misinterpretation of each one of his daughters and his reliance of the â€Å"wolfish† Regan and Goneril, as well as Gloucester’s miscomprehension of Edmund as being a â€Å"loyal and natural boy. The twin paradoxes that appear in the discovery of madness, characterised by Lear, and the discovery of vision in blindness are the most powerful in the development dignity undertaken by Lear and to an extent Gloucester in this play. McAlindon believes that of all the paradoxes it is the fact that th e tragedy develops around an inability â€Å"to contain the worst effects of a terrible eruption in nature,† and none is more terrible than the progressive failing of family links. The typical familial bond of mutual love and affection that is the ornerstone of most families is shockingly absent in King Lear though it is desperately craved by Lear himself. McAlindon believes that this style of bond involves love and justice as well as that it â€Å"predicates a glad and spontaneous performance of offices and responsibilities. † It is therefore bewildering that Edmund would break such a bond in such an anti-familial manner, undermining civilised society in the process, and comparisons can be drawn between him and Iago in his mistreatment of Claudius in Othello. Gloucester, however, can be seen to be as equally to blame for Edmund’s waywardness as his dreadful mistreatment of him, whose breeding is only acknowledged as Gloucester â€Å"often blushed,† and seems to be a victim of tragic causality as Edmund comes to believe that he owes everything to himself. The ancient Greek writers Plato and Aristotle believe that love creates emotional awareness and allows for the creation of just law. With this in mind the manner with which Lear treats the link between him and his daughters in such a material way shatters any sense of order or responsibility in his court with the dismissal of Cordelia and Kent becoming a satire of what passes for justice in society. If Plato’s theories are developed then it would seem that the main cause for injustice is a loss of human kindness and sympathy, explaining fully Edmund’s pathological hardness as his bastardy alienates him. In the same manner in which Edmund suffers a lack of acknowledgement, so too does Lear suffer the same fate, it is only through his mistreatment on the familial ties. McAlindon believes that Lear holds a heavy â€Å"dependence of personal identity on the bond† and it is his reliance on the bond as a material tie makes him a nobody after he divulges himself from his power and estate resulting in one of the most pathetic lines â€Å"I gave you all† separated from the cruelty of Regan. The greatest dignity is then conveyed onto those around him who still perceive the bond to be a union through love, and therefore still hold the same respect for Lear despite his failure to recognise them. The importance of a character understanding the treatment of time plays an integral part in the possibility of them being seen as a tragic figure. King Lear is a tragedy characteristic of its age, a tragedy of extreme and terrible violence, as there is a sense of the untimeliness of violence and destruction that rashness and impatience bring about. Most characteristic of this flaw is Lear as his kingdom implodes through his â€Å"hideous rashness† as he signals the unleashing of pitiless violence that culminates in the utterly pathetic death of Cordelia. He is ironically guilty too of being overly patient as there is an almost comic stichomythia between him and Kent portraying his unwillingness to accept facts. A parallel can be drawn with Gloucester in this as his impatience regarding the supposed traitor Edgar is both unjust and demonstrative of the nexus between time and justice as well as injustice and haste. Calculated swiftness becomes characteristic with the actions of the evil party and can be seen by Edmunds manipulation of Gloucester under the pretence of judicial behaviour as well as that of his brother as he acts â€Å"in cunning† and its placement a the beginning of the line illuminates its two meanings. In an extreme contrast the good party align themselves with time, adopting a policy of patience that is both dignified and tragic. Edgar is keen to wait for â€Å"the mature time† whilst Kent waits for the perfect moment to reveal himself to his master, however, it is his own personal tragedy that he never finds the right moment. This can be seen as a demonstration of a true heart as this is a play that appeals profoundly to the heart as much as it does to the mind. Emblematic of a noble heart is the manner in which a protagonist empathises and treats those around them and powerful contrasts can be seen between characters and their counterparts. Indeed the most powerful of these contrasts is between the â€Å"dog-hearted daughters† of Lear and Cordelia with the scenic juxtaposition of tranquil Dover and the castles and courts of Regan and Goneril a clear demonstration of this. To be truly tragic in King Lear a character possess a good heart and this is perfectly shown by the â€Å"marble hearted sisters† as opposed to Kent’s whose own heart is pierced by Lear’s rejection of Cordelia. Alongside the good characters Lear’s heart is true in its nature, though he seems to suffer the promethean anguish, with his heart replacing the traditional liver, culminating in his death which must be presumed as being from a broken heart. Compassionate love is the supreme value in the play and as discussed above beliefs and social morals come from love and therefore the heart. Conversely though a slighted heart can produce the most devastating fury and hatred through grief as not only does the heart present the duality of nature with the possibility for disunity and anarchy but in this same manner emphasizing the importance of patience. Therefore the presence of all the aforementioned undertones and subtle themes tragedy is both made distinctly more unattainable as well as becoming much more powerful in its nature, with pathos coming to play a key role in its development.

I will begin at the grassroots level, which is primary school, local and junior leagues progressing through county, club and country. The areas included will be funding, incentive schemes, provision for different abilities etc. The local provision of football around my area with the help from my own experiences locally will be compared to the National set-up. Though they are two separate topics they will link together. Review of Literature  In my local area there is large mounts of youth leagues ranging from under 8s competitions upward, the grassroots football. The Kent messenger (KM) newspaper shows the variety of league and cup competition. Youth football in my area ranges from Maidstone Minor Leagues to the Kent League, North, Central and South. The KM and another resource, The Downswood Mail always have advertisements for players to join clubs, or managers that are needed for youth football. The Football Association (FA) consists of the professional and amateur game, with different branches off of it, for example, Kent Football Association. The FA gives you details about coaching schemes and initiatives available to young footballers hoping to progress in the game. The FA also gives details of semi-pro clubs like my club Lordswood who play in the Kent League up to the main, elite premiership clubs. These clubs are local and National clubs to me. Football is getting bigger all the time; a lot of money is pumped through this sport, one example is the à ¯Ã‚ ¿Ã‚ ½45 million pounds the FA is giving to grassroots facilities. Though a lot of this money is given and accumulated by the top clubs. This helps to promote sporting excellence and it gives them a better chance of succeeding. Locally there is the Sports and Play Team, which helps in many ways the progress of football in Maidstone. The Sports and Play team will be setting up Football in the Community with local professional and amateur football clubs in order to create more opportunities for young people to engage in positive activity after school and to develop the clubs. The Sports and Play Team are one of the partners in the Active Sports programme and are keen to work with local clubs. Voluntary Club Facility Hire Grant helps to provide the facilities needed to run training sessions and to have a good quality pitch for match days, ranging form hundreds to thousands of pounds and this scheme was also set up by the Sports and Play team. Local football around my area uses a lot of recreational ground and schools playing fields, so facilities for players are limited. More money needs to be spent on proper catering specifically for football. à ¯Ã‚ ¿Ã‚ ½45 million pounds from the FA is going to be invested in pitches and facilities over the next three years from the National Game Scheme. This does help, but it is not enough for national improvement in this sport. Local amateur football gives a good impression of football nationally; this means that more input in the sport is needed. More progress is being made with issues inside football than with actual provisions for the sport. Schemes such as Fair Play, Eliminating Racism From Football and Child Protection Best Practice do not help with local and national provision. These issues should be addressed but equally with factors like facilities and pitches. Maidstone runs a Youth Sport Action Group developing football with the help of coaches, teachers and members of the community. They organise Youth Sport Achievement Awards and the Sports Fair. Conclusion The FA seems to be the main contributor to grassroots all the way up to the national game. More money is needed for County Councils, such as my local county, Kent. I suggest that schemes to improve facilities for amateur clubs are needed to help bridge the gap between amateur and professional clubs because the breach is currently massive. Money will improve players capabilities because of the enhanced equipment, providing the top clubs with better quality players and the players with a better chance of reaching the top clubs. The more money the FA gives to councils and the Government, the more football will prosper and generate money. If local and national grassroots football improves, more money will be generated by itself, which then takes off all the pressure from the Government and the FA in the future. Appraisal There are lots of ways in which the study could be improved. Without a comprehensive study of another Borough or County its hard to tell if my local area equates in a similar way to another. Many figures that I have found only say what money is going to be put in; it would help to find where the money has been spent so far. Many coaching schemes and building enterprises are organised but the popularity and success is not always shown. My area really only has one professional club, Gillingham, regions in and around London for example will have many clubs reasonably close. This means my area is disadvantaged but I would like to find out whether these major clubs to make a big difference to local and national provision.

Pulse Oximetry For Anaesthesia In Veterinary Practices Biology Essay Pulse oximetry is one of the most commonly used pieces of monitoring equipment for anaesthesia in veterinary clinics today. Using a pulse oximeter allows us to monitor the percentage of haemoglobin (Hb) which is saturated with oxygen in a non-invasive way, allowing us to detect hypoxia before the patient is visibly cyanotic. The pulse oximeter consists of a probe attached to the patient (usually tongue, ear, or prepuce/vulva) which is linked to a computerised unit. The unit displays the percentage of Hb saturated with oxygen and a calculated heart rate, often with an audible signal for each pulse beat. Some units also have a graphical display of the blood flow past the probe called a plethysmograph. The pulse oximeter is able to determine the percentage of haemoglobin saturated with oxygen, commonly referred to as SpO2, by emitting red and infrared light from the light-emitting diodes (LEDs) on one side of the probe, which travels through the tissue (or reflects off depending on the probe type) to the photodiode on the other side of the probe. The machine analyses the light that reaches the photodiode and is able to detect subtle differences in the absorption of light by oxyhaemoglobin and deoxyhaemoglobin. As these differ in absorption levels, the amount of red and infrared light absorbed by blood is related to haemoglobin oxygen saturation. The pulse oximeter can calculate the heart rate as it detects the pulsations as the volume of arterial blood in the tissue changes during the pulsative cycle, affecting light absorption. Adequate oxygenation is essential at all times for the body to perform its metabolic processes. The heart and brain are the bodys biggest consumers of oxygen, and if oxygenation levels decrease to critical levels, tissue damage occurs extremely quickly. Oxygen travels in the blood in two forms as unbound oxygen dissolved in plasma and as oxygen that is bound to the haemoglobin. In healthy patients breathing room air (which contains approximately 21% oxygen), oxygen dissolved in plasma equates to a very small percentage of the total oxygen in the blood (most texts list this as less than 1.5%), and the majority of blood oxygen is bound to haemoglobin (the remaining 98.5%). Measuring and monitoring oxygenation via pulse oximetry is very useful as it is monitoring the oxygen that is bound to haemoglobin, which is what is utilised by the body for normal cell function. Monitoring SpO2 however does not give you a good indication of how well the patient is ventilating (or breathing) for itself, especially during anaesthesia. A common mistake veterinary nurses make is to presume that if a patient has a SpO2 of 95% or higher under anaesthesia, then it is breathing adequately. We can get lulled into a false sense of security by having a good saturation figure when the patients respiration is completely inadequate. There are two main functions of respiration, one is getting oxygen out of the air and into the body, and the other is getting carbon dioxide out of the body and into the air. It possible for the patient to be getting enough oxygen into their body but not being able to get rid of enough carbon dioxide, so the SpO2 will show a good reading, but the patient may be hypercapnic (elevated levels of carbon dioxide). A capnograph should be used to measure end tidal carbon dioxide (ETCO2) levels and assess patient respiration. Partial pressure of oxygen in arterial blood (PaO2) is a measurement of the levels of unbound oxygen in the plasma, and as discussed above, makes up a small percentage of the total oxygen in the blood. However PaO2 is important as it influences the saturation of haemoglobin because there must be an adequate level of dissolved oxygen in the blood to be available to bind to the haemoglobin. It is also important to understand that oxygen saturation and PaO2 are linked (when one goes up the other goes up and vice versa), however it does not have a direct linear correlation. As PaO2 decreases, the saturation level decreases slowly at first, but then decreases rapidly (see table **). In a patient which is breathing room air, the PaO2 is about 100mmHg, whereas for a patient breathing 100% oxygen (as for anaesthesia), their Pa02 is around 500mmHg and SpO2 is 100%. If this patient has a PaO2 drop to 100mmHg (a drop of 400mmHg) their SpO2 will drop to around 98%. If a further drop to 80mmHg occurs, their SpO2 will drop to around 95%. After this point, the SpO2 will start a more dramatic drop; if PaO2 drops to 60mmHg (another 20mmHg drop) will mean their SpO2 will be about 90%. A further drop of 20mmHg to a PaO2 of 40mmHg, the saturation will go from 90% to 75%. In practical application, when monitoring SpO2 in a normal healthy dog or cat, it should be 95-100%. Levels between 90-95% must be investigated, and critical values for oxygen saturation are below 90%.Simply, this means that the total oxygen available to the body decreases very little when partial pressures are above 80mmHg (Spo2 of 95%), however they decrease rapidly below this level, such as patients with lung disease, lack of oxygen, inadequate ventilation etc. Practically put, if you patient has a Sp02 of 90-95% this can indicate hypoxaemia and must be investigated as your patients haemoglobin is not fully saturated. If your patient has a Sp02 of less than 90%, then immediate therapy must be initiated oxygen if not receiving already, ventilation assistance etc. Sp02 of 85% or below for more than 30 seconds is considered an emergency. Placing the SpO2 Probe There are two main types of probes available on the market transmission or reflective. Transmission probes are the most common, and are usually mounted in a clip. These are generally used on the tongue, pinna, toe webbing, vulva or prepuce, or any other area that is thin and relatively hairless. Reflective probes have the light source and sensor side by side and are often taped to the base of the tail after it has been clipped, or covered and inserted into the oesophagus or rectum. When placing rectally, it is important to ensure that there are no faeces between the sensor and the rectum wall. Tongue, Cheeks, Prepuce, Vulva With tongues, start at the tip and work your way toward the base. Always direct the light downward, toward the floor; regardless of the animals position to reduce the effects of ambient light (ambient light will affect accuracy). For patient comfort, keep the tongue moist during longer procedures by applying a dampened gauze swab between the tongue and the probe. Do not have the gauze too thick as it can alter the reading by impeding the light transmission. To get a better reading on smaller tongues, bring the sides of the tongue up and pass the light through both layers. Do not fold the tip of the tongue, as you will restrict blood flow to the tongue. The same principals apply to placing the probe on the cheek, prepuce or vulva. Hock Moisten the hock area with isopropyl alcohol and/or water, and clip hair if needed. Pinna (Ear) The probe can be placed on the ear using the same technique as the tongue. Long haired animals may need a patch shaved first for the sensor to work correctly. Toes Probes can be placed on the metatarsals or metacarpals or in the webbing between them. Tail Place the reflective probe on the ventral base of the tail. The LEDs should be positioned dorsally. You may need to clip a small patch of hair, only large enough for the LEDs to lay on the skin. Be sure the skin is clean. Hold the sensor snugly against the tail and wrap with non-adhesive wrap. Poor SpO2 Readings When you detect a poor or low saturation reading, it is vital that you check the patient before you check the machine. Make sure your patient is stable by assessing all vital signs. Pulse oximeters need a strong regular pulse where the probe is located. If there is only a weak pulse, the pulse oximeter may display a reading but it might not be accurate. Most pulse oximeters have a pulse strength indicator as a bar graph and this should be used to ascertain whether you have correct placement. If the clip of the probe is too strong, this can also affect your reading by constricting the blood flow in front of the sensor. If this is the case, swapping the clip for a more gentler one is the best option, otherwise reposition the sensor to somewhere that can take the pressure (this will usually be thicker). An irregular signal caused by an irregular heartbeat or by the patient moving, shivering or fitting can cause problems for a pulse oximeter. If a patient moves too much, try relocating the probe to another location. Ambient light may be too bright for the sensor to operate correctly. Theatre lights can especially cause issues. Any sensor that is located in bright light should have a drape placed over it to reduce light contamination for more accurate readings. Do not place the sensor is on the same limb as a blood pressure cuff, the blood flow restriction from the cuff during measurement will interfere with the pulse oximeter sensor operating correctly. Other factors that can affect SpO2 readings include pigmented skin either normal pigment or jaundiced patients; peripheral vasoconstriction eg hypothermia, shock, drug-induced; or excess hair can cause interference and should be clipped away to allow the probe to sit directly against the skin. Wetting down with alcohol can also help with excess hair when you are not allowed to clip. Pulse Oximeter Maintenance Read your manufacturers instructions regarding the care of your pulse oximeter and probe. For infection control, you should always wipe the probe sensor and clip between patients. Most sensors can be surface-cleaned by wiping with 70% isopropyl alcohol. Do not immerse the sensor in liquid unless the manufacturer instructions state immersion is possible. After each cleaning and prior to each use, inspect the sensor and cable for fraying, cracking, breakage, or other damage. Inspect the clip for cracking or breakage, or loss of spring tension that would allow slippage or movement of the sensor from its proper position. If defects are noted, do not use the sensor or clip as it may provide an inaccurate reading. When used properly, pulse oximeters are an easy to use and readily available piece of monitoring equipment especially for anaesthesia, however it is important to note that they do not replace hands-on monitoring, and are not a valid method of assessing whether a patients respiration is adequate, as they provide a late indication of respiratory issues.
